Post-trabeculectomy overdrainage resulting in hypotony necessitates surgical intervention if conservative measures fail. This video describes the surgical approach to this complication in a 27 year-old male with traumatic glaucoma. A full thickness scleral patch graft to tamponade the underlying scleral flap is secured with overlay compression sutures. Overlay scleral patch graft compression sutures allow for post-operative intraocular pressure adjustment by laser suture lysis if required. This surgical technique was successful and the patient’s hypotony maculopathy resolved with a final post-operative intraocular pressure of 11mmHg. Informed consent was obtained for video recording.
